Responsibilities:
- Assess potential Hospice or Home Health patients in a timely manner
- Educate patient, family, and caregivers on Hospice and Home Health services
- Conduct patient assessments to determine their needs and develop individualized care plans
- Include the patient and caregiver in the development of the care plan
- Collaborate with healthcare professionals to coordinate diagnostic evaluations and treatment plans
- Provide direction to LPN to ensure patients needs are met
- Utilize knowledge of physiology and medical conditions to provide appropriate care and interventions
- Perform educational visits to potential patients, providers, or facilities as requested
- Participate in IDT and assist in Home Health to Hospice conversions
- Conduct utilization management activities to ensure appropriate use of healthcare resources
- Provide education and support to patients and their families regarding their healthcare options and treatment plans
- Coordinate delivery of needing supplies, equipments, and medications in a timely manner
- Provide education to community partners on hospice and home health and assist in business development
- Perform Case Management responsibilities including but not limited to updating plan of cares, performing recertifications, and supervisory visits.
Skills:
- In-depth knowledge of physiology and medical conditions, allowing for accurate assessment and intervention for both Hospice and Home Health
- Proficient in conducting patient assessments and utilizing diagnostic evaluations to inform care plans
- Excellent patient service skills, with the ability to provide compassionate and comprehensive care
- Experience in hospice and good understanding of qualifications for hospice.
