JOB SUMMARY

The registered nurse plans, organizes, and directs hospice care, emphasizing community health education and experience. As a professional nurse, you will utilize your nursing skills to provide comprehensive and compassionate care to patients and their families within their homes and communities.

DUTIES & RESPONSIBILITIES

Patient Care

• Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs
• Provide professional nursing care using the nursing process
• Develop care plans based on nursing diagnosis and incorporate palliative nursing actions
• Administer medications and treatments as prescribed by the physician
• Assist patients with activities of daily living and promote self-sufficiency and comfort care
• Coordinate patient care as the Case Manager

Communication

• Maintain accurate and relevant clinical notes regarding patient's condition and care
• Communicate with physicians and other healthcare professionals to coordinate the care plan
• Teach patients and families self-care techniques
• Provide a safe environment for patients
• Work with the interdisciplinary group to meet the emotional needs of patients and families
• Attend interdisciplinary group meetings

Additional Duties

• Participate in on-call duties as defined by the on-call policy
• Ensure necessary equipment and services are available
• Supervise ancillary personnel and delegate responsibilities when required
• Maintain and upgrade professional knowledge and practice skills through continuing education
• Actively participate in quality assessment performance improvement teams and activities
