PRN RN Hospice (per diem)

Job Summary
The hospice registered nurse plans, organizes, and directs home care services with a focus on community health education and experience. You will work closely with individuals and families within their homes and communities, utilizing available resources to meet their unique needs.
Duties & Responsibilities
Patient Care
- Develop and initiate personalized care plans in partnership with patients, representatives, and caregivers.
- Implement preventative and rehabilitative nursing procedures as ordered by physicians.
- Provide counseling and education to patients, caregivers, and families.
- Prepare clinical notes and maintain effective communication with the healthcare team.
- Identify discharge planning needs and implement appropriate measures.
Communication
- Update primary physicians and communicate any changes in patients' conditions.
- Coordinate care plans with community health-related individuals.
Additional Duties
- Participate in quality assessment and performance improvement programs.
- Conduct in-service training and provide supervision to home health aides.
- Ensure availability of necessary equipment and services.
- Instruct, supervise, and evaluate home health aide care.
- Supervise Licensed Practical Nurses (LPNs)/Licensed Vocational Nurses (LVNs) and/or Home Health Aids (HHAs)/Certified Nursing Assistants (CNAs), as directed.
Job Requirements
- Graduated from an approved school of nursing and licensed in the state of practice.
- Minimum of two years of experience, with preference given to those with public health or home care nursing experience.
- Current CPR certification.
- Licensed driver with a reliable and insured automobile.
- Self-directed and able to work with minimal supervision.
- Excellent observation, problem-solving, and communication skills.
- Ability to organize and prioritize workload independently.
- Management experience not required.
- Physical ability to perform required nursing skills.
