LPN Clinical Coordinator

JOB SUMMARY 


We are seeking an experienced and compassionate LPN Clinical Coordinator to join us in this mission. The LPN Clinical Coordinator is responsible for ensuring that patient care is coordinated and managed appropriately as well as is responsible for ensuring that care and services are delivered appropriately. 


RESPONSIBILITIES 

  • Ensures compliance with all state, federal, and Joint Commission referral/intake regulatory requirements. 
  • Directs the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy. 
  • Establishes and maintains positive working relationships with current and potential referral sources. 
  • Ensures seamless transition of patients to home care by providing direct oversight of patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers. 
  • Reviews clinical documentation to help determine home health appropriateness. 
  • Assist in preparation of QAPI quarterly.   
  • Attends case conference meetings with home health personnel to facilitate coordination of care and discussion of interdisciplinary group involvement. 
  • Tracks and updates team members on new hospitalizations, hospital holds and resumptions of care. 
  • Communicates with patients, families and caregivers regarding home health services. 
  • Coordinates with physicians and referral sources (as appropriate) regarding home health plans of care and obtains initial orders as needed.
  • Assists in ensuring that patients are seen in a timely manner and works with physicians to receive updated start of care orders when necessary. 
  • Selects, reviews and approves face to face encounter. If necessary access available portals for obtaining updated face to face documentation. Facilitates scheduling of new face to face encounter appointment as needed.
  • Cross trains to provide back-up coverage for scheduling and intake coordination when needed. 
  • Filter calls from field staff as well as help with questions to support field staff, help with stat or urgent needs, Order medications. 
  • Assists Clinical Director and other supervisory home health personnel in the planning, implementation and evaluation of in-service and continuing education programs.  Assists in the formulating, revising, implementing, and evaluating organization policies, procedures, goals and objectives, both short and long range. 
  • Assists in the screening and interviewing process of new employees and makes recommendations for employment of individuals.  Assists in the orientation of new employees. 
  • Other tasks and responsibilities as assigned by the Clinical Manager and/or Executive Director. 

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities) 

  • Graduate of an accredited practical nurse or vocational nursing program. 
  • Three years nursing experience.  Community health/home health or medical/surgical experience is preferred. 
  • Currently licensed as an LPN/LPN in the State. 
  • Complies with accepted professional standards and practice. 
  • Demonstrates good verbal and written communication, and organization skills. 
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. 
  • Possesses and maintains current CPR certification.
