Per Diem On-Call Weekend Registered Nurse

Overview:

The Per Diem On-Call Weekend Registered Nurse will play a crucial role in our organization, providing high-quality care to patients in their homes and communities. Duties & Responsibilities:

• Develop and initiate the plan of care in partnership with the patient, representative, and caregiver(s)
• Provide preventative and rehabilitative nursing procedures as ordered by the physician
• Offer patient, caregiver, and family counseling and education
• Communicate with the physician and other healthcare practitioners
• Identify discharge planning needs and implement prior to patient discharge

Communication:

• Prepare clinical notes and update the primary physician as necessary
• Coordinate care plan with community health-related persons

Additional Duties:

• Participate in quality assessment and performance improvement program
• Participate in on-call duties as defined by the on-call policy
• Supervise home health aides and other staff as directed

Job Requirements:

• Graduated from an approved school of nursing and licensed in the state
• Minimum of two years of experience, preferably in public health or home care nursing
• Current CPR certification
• Valid driver's license and reliable transportation
• Self-directed with excellent observation, problem-solving, and communication skills
• Able to organize and prioritize workload independently
• Ability to lift, position, or transfer patients and supplies
