JOB SUMMARY
The registered nurse plans, organizes, and directs hospice care, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services that meet the needs of individuals and families within their homes and communities.
DUTIES & RESPONSIBILITIES
Patient Care
- Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs.
- Provide professional nursing care utilizing all elements of the nursing process.
- Assess and evaluate patients' status, write and initiate a plan of care, regularly re-evaluate patient and family needs, and participate in revising the plan of care as necessary.
- Use health assessment data to determine nursing diagnosis and develop a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions.
- Initiate appropriate preventive and rehabilitative nursing procedures, administer medications and treatments as prescribed, and provide health care instructions to patients.
- Assist patients with activities of daily living, facilitate their efforts toward self-sufficiency and optimal comfort care, and act as a case manager to coordinate patient care.
Communication
- Complete, maintain, and submit accurate clinical notes regarding patients' condition and care given.
- Communicate with physicians, community health-related persons, and the interdisciplinary group to coordinate the care plan.
- Teach patients and families self-care techniques, provide medication and diet instructions, and work in concert with the interdisciplinary group.
- Provide and maintain a safe environment for patients, assist in providing continuity of care, and meet the emotional needs of patients and families.
- Attend interdisciplinary group meetings.
Additional Duties
- Participate in on-call duties as defined by the on-call policy.
- Ensure arrangements for equipment and other necessary items and services are available.
- Supervise ancillary personnel and delegate responsibilities when required.
- Maintain and upgrade professional knowledge and practice skills through continuing education and in-service classes.
- Actively participate in quality assessment performance improvement teams and activities.
JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
- Graduate of National League for Nursing accredited school of nursing.
- Minimum of two years of experience, with at least one year in public health, home care, or hospice nursing preferred.
- Management experience not required. Responsible for supervising hospice aides.
- Registered nurse with current licensure to practice professional nursing in the state.
- Demonstrates excellent observation, problem-solving, verbal and written communication skills, and nursing skills per competency checklist.
- Shows ability to organize and prioritize workload independently.
- Prolonged or considerable walking or standing. Able to lift, position, and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, and/or crouching. Visual acuity and hearing to perform required nursing skills.
- Maintains a current CPR certification.
- Must be a licensed driver with an insured automobile in good working order.
- Self-directed and able to work with minimal supervision.
