JOB SUMMARY
The Occupational Therapist contracted through our organization will be responsible for implementing standards of care for occupational therapy services and ensuring adherence to all conditions outlined in the Service Agreement.
DUTIES & RESPONSIBILITIES
- Assume responsibility for ongoing interdisciplinary assessment and development of individualized care plans in partnership with patients, representatives, and caregivers.
- Provide services as ordered by physicians and indicated in the plan of care.
- Offer patient, caregiver, and family counseling.
- Deliver patient and caregiver education.
- Communicate with physicians and other healthcare practitioners to coordinate care.
- Evaluate patients' functional status and home environment, identifying equipment needs and hazards.
- Develop treatment programs and establish goals for improved function.
- Train patients in new skills or retrain them in daily activities.
- Fabricate splints and instruct patients in the use of adaptive equipment.
- Train patients in the use of prosthetic and orthotic devices.
- Maintain clinical records and report any changes in patients' condition.
- Participate in quality assessment, performance improvement programs, and in-service training.
- Identify patient and family needs for additional home health services and make appropriate referrals.
- Instruct patients' family/caregivers and other healthcare personnel in treatment regimes.
- Supervise Certified Occupational Therapy Assistants according to organization policy and state regulations.
- Instruct, supervise, and evaluate home health aide care when therapy is the only skilled service.
- Participate in discharge planning for patients.
JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
- Must be licensed in the location where the HHA is located, if required.
- Graduated from an OT program accredited by ACOTE of AOTA or any successor organization.
- Eligible for or have taken the OT certification examination administered by NBCOT.
- Two years of clinical experience as an occupational therapist.
- Good verbal and written communication skills.
- Strong organization skills.
- Current CPR Certification.
- Licensed driver with an insured automobile in good working order.
