Overview:
The home health registered nurse plans, organizes, and directs/coordinates home care services. This role requires experience in nursing, with an emphasis on community health education/experience. As a professional nurse, you will utilize the resources of the community to plan and direct services that meet the needs of individuals and families within their homes and communities.
Duties & Responsibilities:
- Develop and initiate the plan of care in partnership with the patient, representative, and caregiver(s)
- Provide preventative and rehabilitative nursing procedures as ordered by the physician
- Offer patient, caregiver, and family counseling and education
- Prepare clinical notes and communicate with the physician and other healthcare practitioners
- Identify discharge planning needs and implement prior to patient discharge
- Communicate with the primary physician and community health related persons to coordinate the care plan
- Participate in quality assessment and performance improvement programs
- Supervise home health aides and evaluate their care
Job Requirements:
- Graduated from an approved school of nursing and licensed in the state of practice
- Minimum of two years of experience, with preference given to public health or home care nursing
- Current CPR certification
- Valid driver's license and insured automobile in good working order
- Self-directed and able to work with minimal supervision
- Excellent observation, problem-solving, and communication skills
- Ability to organize and prioritize workload independently
- Prolonged walking or standing, lifting, and physical agility
