Overview:
The Nurse Practitioner is a registered professional nurse with advanced graduate education and clinical training who provides comprehensive hospice services under the supervision of a collaborating physician. In this role, you will evaluate and assess health history, promote health, diagnose medical conditions, manage health problems, prescribe medications or treatments, and coordinate care with patients, families, and other healthcare practitioners. You will also visit patients, perform physical assessments, and determine hospice eligibility in consultation with the interdisciplinary team.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES:
Patient Care:
- Provide medical care, palliation, and management of terminal illness in compliance with the state Nurse Practice Act
- Conduct history and physical examinations, interpret test results, diagnose health conditions, and document findings
- Develop individualized plans of care, prescribe medications or treatments, coordinate services, and ensure symptom control
- Complete face-to-face visits to assess hospice eligibility and provide clinical information for recertification
- Collaborate with the interdisciplinary team to review and revise the patient's plan of care
- Communicate and coordinate with attending physicians, healthcare practitioners, and community resources
- Educate patients, families, and caregivers on preventive care, medical and psychological problems, and spiritual needs
Additional Duties:
- Maintain knowledge of and compliance with current Medicare/Medicaid and state/federal regulations
- Ensure compliance with Medicare conditions of participation and other healthcare regulations
- Comply with HIPAA requirements and maintain accurate documentation
- Participate in quality improvement initiatives and professional development activities
POSITION QUALIFICATIONS:
- Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program
- Current nursing licensure in State and CPR certification
- Master's degree with a minimum of one (1) year Nurse Practitioner experience and hospice or palliative care experience
- Accreditation from an approved certifying body for advanced practice nursing
- Certification in a specialist area preferred (e.g., Hospice and Palliative Nursing, Pediatrics, or Geriatrics)
- Excellent observation, communication, problem-solving, and nursing skills
- Ability to lift, position, and transfer patients, as well as perform required nursing skills
- Valid driver's license and insured automobile in good working order
