Speech Language Pathologist
POSITION SUMMARY:
As a Home Health Speech Language Pathologist, you will make home visits to assess and treat speech, language, social communication, cognitive-communication, and swallowing disorders. You'll be responsible for providing detailed notes and documentation regarding your home therapy visits and patient progress.
RESPONSIBILITIES:
- Knowledge of the principles and practices of speech pathology and provides speech therapy services in accordance with state practice act
- Perform an initial assessment and evaluation of the patient and establish a plan of care, including scheduling subsequent visits determined by the SLP to be clinically appropriate.
- Coordination of care by care conferencing with other health care team members
- Observe, record and report to Primary Care Physician the patient’s treatment, response and progress including significant changes
- Obtain orders to revise plan of care as clinically appropriate
- Perform re-evaluations of patient’s progress at least every thirty (30) days and updates plan of care as necessary
- Complete comprehensive assessments and OASIS data as necessary
- Teach patients and caregivers about the patients’ impairments and provide support and educational materials to support interventions and foster independence
QUALIFICATIONS:
- Master's degree in Speech Language Pathology
- Active Speech Language Pathology license (CCCs)
- Ability to practice speech therapy unencumbered within state and federal guidelines
- At least one year of experience working as a Speech Language Pathologist (no CFYs)
- Home health and Home Care Home Base documentation experience
