Registered Nurse (RN)
POSITION SUMMARY:
You will develop a plan of care in collaboration with the patient, family/caregiver and physician orders including frequency and duration of nursing visits, specific care or treatments to be provided, appropriate goals, medications, and other disciplines needed. You will make regular visits to monitor the patient’s condition, assess their wounds, provide wound care, and other nursing responsibilities to patients with chronic conditions, illnesses, or injuries
RESPONSIBILITIES:
- Perform comprehensive assessments in the patients’ homes and, identifies patient’s needs
- Develop a plan of care in collaboration with patient/CGs, Health Care Provider, case manager, and /or Supervisor as assigned and document appropriately
- Assess the need for additional services and equipment on admission and on an ongoing basis and inform the case manager and/or supervisor on a timely basis.
- Assess the home environment and make recommendations that will enhance patient care, promote safety, and prevent adverse effects
- Take verbal orders from the Health Care Provider as necessary in a timely manner and documents appropriately
- Initiate appropriate preventive, rehabilitative, therapeutic, comfort, and pain interventions and provide instructions to patient/CG’s and staff in a timely manner
- Demonstrate professionalism by maintaining high standards of clinical care and staying current on clinical and medication issues
- Updates to patients’ clinical records including medication profiles, plan of care, OASIS, case conference, progress notes and re-certification documentation as required in a timely manner
QUALIFICATIONS:
- Graduation from an accredited nursing program, current state license as a Registered Nurse, and CPR certification.
- Has at least one year of clinical practical nursing experience. Home Health experience preferred.
- Must have a vehicle and current driver’s license and automobile insurance
- Interpersonal and communication skills necessary
- Ability to make home visits and provide skilled nursing assessments and care in the home
