Hospice RN- Case Manager

The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community, health education/experience.  The

professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.

  • Position Description/Qualifications:

Full-time and Part-time RN positions working in Mesa County, and the surrounding areas providing hospice care. Competitive pay rates discussed upon interview. Mileage reimbursement, work phone/charting device plus other employee benefits and perks! Current RN license required. Applicants should have excellent customer service skills, be comfortable working independently, Case managing a patient's care in the home setting and working in an EMR charting system. JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)

1. Graduate of National League for Nursing accredited school of nursing.

2. Minimum of two years of experience, at least one of which is in the area of public health,

home care, or hospice nursing is preferred.

3. Management experience not required. Responsible for supervising hospice aides.

4. Registered nurse with current licensure to practice professional nursing in the state.

5. Demonstrates excellent observation, problem solving, verbal and written communications.

6. Shows ability to organize and prioritize workload independently.

7. Prolonged or considerable walking or standing. Able to lift, position, and/or transfer

patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending,

kneeling, and/or crouching. Visual acuity and hearing to perform required nursing skills.

8. Maintains a current CPR certification.

9. The ability to drive and/or have access to transportation for agency-related travel (e.g.

community partners, patients, etc.) is an essential function of the job. Must have and maintain

a valid driver’s license, maintain automobile insurance coverage and have access to an

automobile.

10. Self-directed and able to work with minimal supervision.
