Dual Certified Psychiatric Nurse Practitioner

Job Summary:


Whole Person Care as a dual Psychiatric Mental Health-Family Nurse Practitioner allows one to use an essential body of knowledge and skillset to address various common health problems, across health settings, in this person-centered optimal use of diverse healthcare resources to deliver the physical, behavioral, emotional, and social services required to improve care coordination, well-being, and health outcomes. Integrated care for persons with chronic illnesses and moderate to serious mental illnesses acknowledges the interconnectedness of physical and mental health and aims to improve their overall health and well-being by providing comprehensive, coordinated, and patient-centered care. The care recognizes that treating these conditions in isolation might not yield optimal outcomes and that a holistic approach is essential for better health outcomes and improved quality of life.


A Psychiatric Mental Health-Family Nurse Practitioner treats illness and partners with patients to help them achieve their own mental health and wellness goals. The practitioner is an essential member of the primary care team for patients with complex mental health and chronic care needs that becomes crucial to the comprehensive treatment of individuals. Mental health and substance use disorders are among the most common disabling health problems worldwide and contribute to the presenting complaints in a high proportion of care visits. The complex physical and mental health needs of persons with serious mental illness and substance use disorders are well recognized, with most patients with psychiatric-mental health and substance use disorders experiencing comorbid medical illness. It is also vital development that effectively manages coexisting physical and mental health disorders to meet the growing needs of the population practicing in the community context.


Duties and Responsibilities:

  • Focus on the integration of behavioral healthcare methods in primary care settings
  • Manage complex health problems with evidence-based, person and family-centered care focus on complex health Integrated care for people with chronic illnesses and moderate to serious mental illnesses refers to a comprehensive and coordinated approach to healthcare that addresses both their physical health needs and mental health needs in a holistic manner
  • Practice and collaborative approach using integrated care involving collaboration among healthcare professionals from various disciplines, such as primary care physicians, mental health specialists, nurses, enabling specialists, and therapists
  • Conduct a person-centered assessment through an approach that considers physical, cultural, social, environmental, spiritual, health, mental health conditions and strengths of the individual
  • Coordinate integrated care to ensure communication and coordination among different healthcare providers is seamless
  • Encourage Shared Decision-Making for individuals to actively participate in their care planning process
  • Plan and develop treatment plans with a focus on addressing both chronic physical illness and mental health condition
  • Conduct screening and early intervention with regular screenings conducted to identify any changes or developments in the
  • individual's physical and mental health status
  • Provide education and support patients and their families to better understand the relationship between physical and mental health
  • Aim to provide access to services including medical care, psychotherapy, medication management, and social services
  • Follow-up and monitor regular appointments to track progress, adjust treatment plans as needed, and address any emerging
  • issues promptly

Required Skills/Abilities:

  • Ability to respond effectively to the most sensitive inquiries or complaints
  • Ability to handle multiple projects simultaneously
  • Ability to read, analyze, and interpret complex documents

Preferred Skills/Abilities:

  • None

Required Education and Experience:

  • Credentials in both Family Nurse Practitioner and Psychiatric Mental Health Nurse Practitioner
  • Licensed as a Registered Nurse with recognition as Advanced Practice Nurse
  • Master of Science in Nursing
  • Certification in area of practice or eligibility for certification
  • ACLS certification desirable

Preferred Education and Experience:

  • Experience in the healthcare fields (primary or acute care)
  • Experience working with non-profit organizations
