Behavioral Health Group Access Coordinator

MyCHN is a nonprofit organization providing Medical, Dental, Behavioral Health, and Pharmacy services to communities in the South Houston and Gulf Coast area. We were founded in 2008 with one provider, three exam rooms and a big dream. Today, we have over 100 providers and more than 15 locations. Job Summary:


This position focuses on coordinating, supporting, and enhancing access to behavioral health group care services. The position will assist in enrolling patients in suitable groups while working closely with patients, families, and community resources to help ensure patient's access to group care while providing comprehensive, whole-person care.


Duties and Responsibilities:

  • Coordinate group programs and group sessions.
  • Provide whole-person care services to patients.
  • Assist with patient transportation scheduling
  • Collaborate with providers to enhance participation in group sessions.
  • Track and report on program metrics and patient referrals.
  • Support the establishment of peer support groups
  • Follow up on missed group sessions.
  • Conduct marketing and outreach to promote behavioral health group expansion.

Key Performance Indicators (KPIs):


KPI 1: Number of Group Programs Coordinated

  • Definition: The total number of behavioral health programs and sessions coordinated during a given period (weekly, monthly, quarterly)
  • Target: Coordinate at least 10-12 group programs per month, depending on the program
  • structure and community needs

KPI 2: Group Session Attendance Rate

  • Definition: Percentage of patients attending their scheduled group sessions out of those
  • who were enrolled
  • Target: Achieve an attendance rate of 80-90% for all scheduled group sessions

KPI 3: Average Group Size

  • Definition: The average number of participants attending each group session
  • Target: Maintain an average group size of 8-10 participants per session

KPI 4: Group Program Enrollment Growth Rate

  • Definition: The percentage increase in the number of participants enrolled in group programs compared to the previous year or quarter
  • Target: Achieve a 10-15% growth in group program enrollment annually

KPI 5: Program Sustainability and Funding

  • Definition: Number of successful grants, partnerships, or funding resources secured to
  • support group care services
  • Target: Secure at least one new funding source (e.g., grant, donation, partnership) annually to sustain and expand group care services

Required Skills/Abilities:

  • Proficiency in scheduling software and tools        
  • Strong analytical and problem-solving abilities
  • Excellent communication and interpersonal skills
  • Ability to work independently and manage multiple tasks efficiently
  • Understanding of behavioral health services and best practices in scheduling and access

Preferred Skills/Abilities:

  • Bilingual ability in Spanish

Required Education and Experience:

  • High school diploma or GED equivalent

Preferred Education and Experience:

  • Clerical or front office experience in a medical or dental facility
