We were founded in 2008 with one provider, three exam rooms and a big dream. Today, we have over 100 providers and more than 15 locations. Job Summary:
The Psychiatric Mental Health Nurse Practitioner role is providing primary mental health care services, to those with mental health problems, or psychiatric disorders. The PMHNP is required to assess, diagnose, provide treatment plans, prescribe medication therapy, and offer counsel across the lifespan. The PMHNP provides care in a wide range of settings to children, adolescents, adults, the elderly, and their families. This position will be collaboratively managed by the Chief Medical Officer and a staff psychiatrist. The position will be following Patient Centered Medical Home practices.
Duties and Responsibilities:
- Collaborate with other departments (behavioral health, medical, and pharmacy) to integrate care and treatment
- Assist in the development of the Patient-Centered Medical Home Model by managing the care of patients in a manner that balances quality and cost
- Complete the 24-hour program to attain your opioid treatment waiver, by the end of onboarding
- Assist in the development of the Patient-Centered Medical Home (PCMH) Model by managing the care of patients in a way that balances quality and cost
- Participate in Continuous Quality Improvement efforts to achieve goals driven by value-based care
- Timely closing of patient charts, per CHN policy
- Daily production with an average of 20 patient encounters per day
- Conduct comprehensive medical and social history of individuals, including those who are healthy and those with acute illnesses and chronic disease after informed consent obtained
- Make independent decisions regarding management and treatment of medical problems by using evidence-based guidelines, including prescribing medications
- Provide information, instruction, and counseling on health maintenance and prevention, nutrition, health promotion, social problems, illness prevention, and management and medication use

Required Education and Experience:
- Licensed as Registered Nurse with recognition as Advanced Practice Nurse
- Master of Science in Nursing
- Certification in area of practice or eligibility for certification
Preferred Education and Experience:
- ACLS certification desirable
- Experience in the health care field (primary or acute care)
- Experience working with non-profit organizations
