Responsibilities:
- Provide physical therapy services to patients accordign to a physician's written plan of care, assessing and evaluating therapeutic/rehabilitative/functional status and participation in the patient's complete plan of care.
- Determine patients' needs and direct physical therapy treatment.
- Timely documentation (point of service)
- Instruct, motivate and assist patients in improving through functional activities
- Participate in weekly case management meetings to coordinate and promote efficient care.
- Supervise physical therapy assistants according to company ploicies & procedures, and state regulationsd.
- Perform all other duties as assigned.
Requirements:
- Excellent interpersonal and communication skills
- Ability to work with other disciplines in an outpatient setting
- FL PT License
