Position Summary:
LPT’s provide direct patient care and perform a range of nursing tasks focused on patient comfort and mental wellbeing. LPT's may supervises the day-to-day care activities performed by Certified Nursing Aides (CNA) and/or Nursing Assistants (NA). All care and supervisor duties must be executed in accordance with license held and all company policies, current federal, state, and local standards, guidelines, and regulations that govern the industry, ensuring the highest degree of quality care is maintained at all times.
Required Education and Experience:
1. Graduate of an accredited Licensed Psychiatric Technician (LPT) program
2. Current license from California State Board
3. On-the-job training provided
4. Proven ability to remain professional in stressful situations
Responsibilities:
1.Observes, plans, implements, monitors, documents, evaluates and reports on patient care, and mental health as needed or directed
2. Leads by example creating a safe, welcoming and calm work environment
3. Completes and files required recordkeeping (forms, charts, etc.), within the scope of licensure, upon patient’s admission (intake), transfer, change in condition and/or discharge
4. Transcribes physician’s orders to patient charts, systems, medication cards, treatment/care plans,etc. as required communicating with care staff to ensure orders are understood and implemented
5. Reviews care plans daily to ensure that appropriate care is being rendered
6. Makes daily rounds to ensure nursing service personnel are performing their work assignments timely, accurately and in accordance with all acceptable nursing standards and policies
7. Assists in Activities of Daily Living helping patients, developing relationships and assessing, documenting reporting on patient wellbeing and mental health
8. Assists in training and developing CNA/NA skills – creating, planning conducting and scheduling various in-service training classes to ensure a well-education nursing service department
10. Manages and administers medications within the scope of license, training and state regulations
11. Documents patient assessment findings (psychological, psychosocial, etc.), needs and responses to nursing interventions on patient mental health and wellbeing progress
12. Communicates with patients and/or family members as necessary or directed within the scope and limitations of the position
13. Reports patient problems or changes in condition to the appropriate nurse or doctor
14. Consults with the patient’s physicians in providing care, treatments, needs and responses, etc.
16. Assists the R.N., Director and/or other care providers in evaluating patient mental health and wellbeing (psychological, psychosocial, etc.)
17. Ensures an adequate stock level of medications, medical supplies, equipment, protective gear, etc., is maintained at all times to meet patient needs
18. Maintains confidentiality of all patient care information including protected health information. Reporting known or suspected incidents of unauthorized disclosure of such information.
19. Attends mandatory meetings and other duties as assigned
